TLC Advance Directives

Advance Directives provide an important way to communicate health care wishes to family and the medical community. These documents should be completed regardless of age or health status.

For Arizona residents, please refer to the excellent information provided on the Arizona Attorney General’s website at http://www.azag.gov/life_care/index.html. On this site, you can print:

• Durable Health Care Power of Attorney
• Durable Mental Health Care Power of Attorney
• Living Will
• Letter to My Agent
• Prehospital Medical Care Directive (Do Not Resuscitate) form.

Sometimes, the hardest part of completing advance directives is discussing these difficult topics with those people closest to you. For a wonderful resource to assist with this process, please refer to the Rhode Island Health Literacy Project, Your Life Your Choices at www.rihlp.org/pubs/Your_Life_Your_Choices.pdf. Print the entire booklet and complete the workbook pages that address decisions related to specific health care diagnoses and treatments.
