Kathryn Tynan, MSN, A/GNP

Kate Tynan, A/GNP, has been with Tucson Long-Term Care Medical group since its inception in 1997. Kate did her undergraduate work in nursing at the University of Delaware. She worked for several years as a traveling nurse throughout the country. She relocated to Tucson in 1994 to attend the University of Arizona adult geriatric nurse practitioner program and earned her masters in nursing. Kate began her nurse practitioner career as the nurse practitioner for Handmaker Jewish Services for the Aging. She has worked for TLC for the last 8 years and has provided care for patients in almost every skilled nursing facility in Tucson and Green Valley. Kate is more recently involved with starting up a Coumadin clinic within the practice and QA.


Christina Valenta, A/GNP, MBA

Chris Valenta, A/GNP, began her nursing career in 1974 in Illinois. Employed with TLC HealthCare™ Companies since 2000, she brings many years of clinical and administrative experience to the practice. Chris has worked for several medical groups throughout Pima County that focus on geriatric care. Specializing in chronic pain issues and wound care management, she also assists Dr. Bolhack in his outpatient clinic in Oro Valley. She is currently enrolled as a Doctoral student at the University of Arizona.


Hugh Christensen, PA-C

Hugh Christensen joined TLC in 1997. He completed his bachelor of Medical Science degree from the University of Nebraska and was selected to attend the Air Force Physician Assistant Training Program in 1974. He received his degree and certification as Physician Assistant in 1976 and later that year was commissioned as an Air Force Officer. Hugh has practiced in all phases of adult and pediatric medicine, including emergency care disaster planning. He has been honored with an Air Force Commendation and Meritorious Service Medal for his outstanding contributions in the areas of patient care, patient and staff education, and community service.


Beth Orenduff, FNP/BC

Beth Orenduff joined TLC in 2003. In addition to being a Family Nurse Practitioner (FNP), Beth also has the following degrees: Master of Nursing, Masters of Science in Planning, Bachelor of Science in Public Administration. Prior to joining TLC, Beth performed extensive clinical rotations at the University of Arizona in a variety of settings. In addition, Beth's prior roles include case Management, Director of Home Health and Hospice, and Human Resources.



Kathy Sherman, GNP

Kathy Sherman graduated from The Massachusetts General Hospital Institute of health Professions with a Masters Degree in Nursing in 1994 and has been an ANCC certified Geriatric Nurse Practitioner for 13 years. Prior to moving to Arizona in 2003 and joining TLC Kathy worked in the nursing home setting and in home care for Boston City Geriatrics and Home Care, Boston University Geriatrics Service and Hampden County Medical Group. She has a special interest in psychiatric and behavioral problems associated with chronic mental illness and dementia, as well as managing chronic illness.


Shu-Fen Wung, RN, PhD, ACNP, BC

Dr. Wung joins TLC with impressive academic credentials. She is currently an Associate Professor, University of Arizona, College of Nursing. In her academic role, Dr. Wung is conducting cardiovascular clinical research. During her training, Dr. Wung completed clinical rotations in cardiology and intensive care/internal medicine. She earned her Acute Care NP degree and her PhD. Degree from the University of California–San Francisco. She has presented often on cardiac care issues and is published in a number of journals and publications.

Therese "T" Henn, GNP-C

"T" Henn comes to Tucson from South Dakota where she earned her Bachelor's Degree in Nursing in 1975. Her Geriatric Nurse Practitioner degree was awarded from the University of Arizona in 1982. She joins TLC in a collaborative effort with the Pima Health System which she has done since 1984. Her areas of interest and particular expertise include nursing home care and chronic wound care. She is a member of the Tucson Wound and Ostomy Care Nurse's Association.


Susan Mani, A/GNP

Susan comes to TLC after spending the last five years in Cochise County, Arizona. While in Southeast Arizona, Sue worked in a primary care clinic setting and also did nursing home rounds. Prior to that, Sue was part of the Surgical ICU-Heart Team at St. Mary’s Hospital in Tucson. She has also spent extensive time conducting research and providing care in places like Nepal, Honduras, and El Salvador. She has completed internships in Primary Care, Psychiatry, Acute Care, and Hospice. Sue is in the process of completing a Doctor of Naturopathy program.


Neva Crogan, PhD, CNS-BC, GNP-BC, FNGNA

A systems approach to improving the nutritional status and quality of life for nursing home residents is the nursing and research focus for Dr. Neva Crogan (Pomilla). Dr. Crogan received her PhD in 1998 from Washington State University. She is a master’s prepared gerontological clinical nurse specialist (ANCC certified), a geriatric nurse practitioner and a nursing home expert with extensive hands-on experience. Dr. Crogan first worked as a nurse educator training nursing home staff in the care of older adults; then as a Washington State nursing home inspector ensuring quality care and compliance with state and federal regulations. Dr. Crogan also has led a nursing department as Director of Nursing Services in a 120-bed skilled nursing facility. Currently, she teaches and conducts Gerontological research, acts as a consultant and expert reviewer for nursing homes and attorney groups, and serves as Co-Editor of NGNA’s section of Geriatric Nursing. Dr. Crogan’s scholarly work includes more than 40 peer-reviewed articles addressing nutrition care of older adults in nursing homes plus cutting edge research that addresses the current and future needs of older adults living in nursing homes.


Anna DeSimone, MSN/FNP-C

Anna returns to TLC HealthCare™ Companies with experience working in the acute care setting as a hospitalist provider where she rounded on patients on the medical, surgical and ICU units. Anna has a Master of Science degree in Nursing and earned her family nurse practitioner certification from the University of Phoenix in 2003. She has previous nursing experience in the acute rehabilitation and hospices arenas. In addition to her role with TLC providing care to patients in skilled nursing facilities, Anna also provides primary care services to patients at an acute psychiatric inpatient setting.


Jennifer Ramin, MSN, APRN-BC, ACHPN

Jennifer joins TLC with extensive pain and palliative medicine experience, providing care for patients at home, in the hospital, and in many community settings. Before she was a nurse practitioner, she practiced as a hospice nurse in Cambridge, Massachusetts. She trained at the Massachusetts General Hospital, Institute of Health Professions in Boston, MA. Her previous practice emphasized care of patients with HIV/AIDS. Fluent in Spanish, she has a special interest in cross-cultural issues. Ms. Ramin is also adjunct faculty at the University of Arizona College of Nursing.


Virginia LeBaron ACNP-BC, AOCN

Virginia began her nursing career as a clinical staff nurse at the National Institutes of Health in Bethesda, Maryland where she worked first on a neuromedical/neurosurgical unit, and then with adult oncology and bone marrow transplant patients. She received her BSN from the University of Virginia and her Master’s from the University of Maryland. Before relocating to Tucson this past July she worked as the nurse practitioner with the hospital based palliative care team at Georgetown University Hospital in Washington, D.C. She currently holds a joint appointment with University Medical Center and the University of Arizona College of Nursing, and has been actively involved with the development of the palliative care education initiatives and pilot project at University Medical Center. She also volunteers with the International Network for Cancer Treatment and Research, helping to facilitate palliative care education in developing countries.


Jody Agins, MSN, APRN-BC

Jody comes to TLC from Rhode Island where she worked as a family and geriatric nurse practitioner. Jody earned her Master of Science in Nursing from the University of Kansas and her Bachelor of Science in Nursing from Webster University in Kansas City. She is board certified as an adult, family and geriatric nurse practitioner. At Truman State University in Missouri, she was an adjunct and full time faculty member in the nursing department. Jody is on the Board of Directors with Nurse Practitioner Associates for Continuing Education (NPACE) and she has an extensive community service background from children to Alzheimer's issues. She enjoys lecturing on a variety of topics and will be seeing patients in multiple locations for TLC.


Yuri Safronov, NP

Yuri recently completed the FNP program at Northern Arizona University. His immediate prior position was with UMC as one of their staff RNs (2/03-10/06). In that capacity, he worked in Telemetry, Med/Surg, Trauma, Ortho, and Kidney Transplant. Prior to that position, he was employed by Handmaker (7/99-9/03) and worked on their skilled unit and in their inpatient hospice unit. Yuri was a medical doctor in Latvia prior to moving to the US in 1998. His wife is an anesthesia resident at the U of A. Yuri will be full-time with TLC and will be caring for patients at La Colina and in Adult Care Homes.


Gregory Stevenson, FNP-C, BS, MSN, MC

Gregory earned his Master’s of Science in Nursing and Family Nurse Practitioner from the University of Phoenix in 2006. He also earned a Master of Community Counseling in 2000 and a Bachelor’s of Science in Microbiology and Immunology from the University of Arizona in 1997, after he completed his undergraduate education as a Registered Nurse from the University of New York. Gregory has an extensive military background having served in the United States Air Force from 1980 through 1991. He spent time in the Middle East as part of Desert Storm and Desert Shield as well as in remote areas of South America and Southwest Asia. Prior to earning his degree as a nurse practitioner, Gregory worked as a Registered Nurse in a Tucson skilled nursing facility caring for patients on ventilators. He has significant experience in home health care. Additionally, he has worked in the ICU/CCU and ED setting at local hospitals and USAF hospitals. For TLC, he will be caring for patients in skilled nursing facilities.
