Nurse Practitioner - TLC PAIN & PALLIATIVE MEDICINE PRACTICE

This nurse practitioner will be part of a team of clinical providers delivering pain and symptom management and end-of-life care. The ideal candidate will have prior practice experience in a hospice and/or palliative medicine program. The nurse practitioner should be comfortable providing care in various settings including hospice inpatient units, acute care hospitals, Skilled Nursing Facilities (SNF), Assisted Living Facilities (ALF), adult care homes (ACH), and personal residences.
