Tucson Long-Term Care Medical Group, Inc. was formed by Scott M. Bolhack, MD, MBA, CMD, CWS, FACP, FAAP in 1997. Currently, the group comprises more than 6 physicians and 13 nurse practitioners. The providers care for patients in nursing homes, assisted living facilities, and adult care homes in Tucson, Green Valley, and other surrounding communities.
