Tucson Long-Term Care Medical Group, Inc. was formed by Scott M. Bolhack, MD, MBA, CMD, CWS, FACP, FAAP in 1997. Currently, the group comprises more than 10 physicians, 12 nurse practitioners and physician assistants. The providers care for patients in nursing homes, assisted living facilities, and adult care homes in Tucson, Green Valley, and other surrounding communities.
