Overview: Our Many Facets
Scott M. Bolhack, MD, MBA, CMD, CWS, FACP, FAAP founded Tucson Long-Term Care Medical Group, Inc. (now known as TLC HealthCare™ Companies) in 1997. Locally owned and operated, TLC HealthCare™ Companies specializes in caring for geriatric patients in a variety of settings.
The spectrum of living environments for the elderly has expanded over the past decade to include skilled nursing facilities, adult care homes, assisted living facilities, retirement communities, dementia communities, hospice and intense in-home support.
The medical professionals of TLC HealthCare™ Companies have dedicated themselves to delivering primary medical services to patients in these diverse settings. As the needs of the patient change, our medical staff can continue to provide treatment at several locations, ensuring continuity and quality care.
Our family of companies now includes TLC Medical Group - Oro Valley (our primary care clinic formerly known as Scott Matthew Bolhack, MD, PC), TLC Tucson Long-Term Care Medical Group, TLC Pain & Palliative Medicine Practive, TLC Wound Care Services, TLC HealthCare™ Quality Improvement Organization and the newest primary care office TLC Medical Group - Catalina Foothills
There is one common theme to our organization’s evolution. It is based on the question that we continually ask ourselves . . . . How can we improve the quality of care along the continuum for our elderly and for our long-term care residents? Our coordination of care from the hospital or clinic to the assisted living facility to the nursing home to the inpatient setting is probably unmatched by any other medical group in the country. This all points to our involvement in and development of a true chronic care model. We are very proud to be a local, provider-driven healthcare organization. Our business model for physicians and nurse practitioners is structured in such a way that allows them to practice in the settings that they prefer with minimal operational concerns. We pride ourselves in our ability to offer a coordinated and streamlined process for delivering quality medical care. Our customers include our patients, their families, the long-term care facilities and homes, referring physicians, health plans, hospice companies, home health companies, and hospitals.
