Scott M. Bolhack, MD, MBA, CMD, CWS, FACP, FAAP

Scott Bolhack, MD, MBA, CMD, CWS, FAAP, FACP, has been in practice in Tucson since 1992. He began his private practice in Catalina, Arizona, and maintains an outpatient practice in Oro Valley in the Northwest Medical Center at Rancho Vistoso. Presently, he holds the following positions: chief executive officer (CEO) of the Tucson Long-Term Care Medical Group, Inc; CEO of TLC Pain & Palliative Medicine Practice, Inc; president and CEO of TLC Medical Consulting Group, LLC; and president and CEO of TLC HealthCare™ Quality Improvement Organization, Inc. He is a clinical assistant professor with the Department of Medicine at the University of Arizona. Tucson Long Term Care Medical Group, Inc. was awarded the winner in the Business Growth Category 1-19 employees 6th Annual Wells Fargo Copper Cactus Awards in 2003; TLC HealthCare Companies now has over 55 employees.

Dr. Bolhack speaks nationally on the subject of rehabilitation in nursing homes and has gained extensive experience in the area of geriatrics. Dr. Bolhack has additional experience as a medical director for nursing homes, assisted living facilities, hospices, and home health agencies. He is a fellow of the American College of Physicians and a member of numerous professional societies, including the American Medical Directors Association, American College of Physician Executives, American Academy of Home Care Physicians, and American Academy of Hospice and Palliative Medicine.

He has presented over 20 scientific posters in the areas of quality improvement, pressure ulcers, and subacute care in skilled nursing facilities.

Dr. Bolhack earned his bachelor of science in biology from Bucknell University, his doctor of medicine from the Health Sciences Center of Brooklyn, and his masters in business administration from the Eller College at the University of Arizona. He is board certified in internal medicine, hospice and palliative medicine, and pediatrics. He has additional credentials as a certified wound specialist.

Paul Tatum, MD, CMD

Dr. Tatum is board certified in family practice and hospice and palliative medicine. In addition, he holds a Certificate of Added Qualification in Geriatric Medicine. Prior to joining the group, Dr. Tatum was a Senior Clinical Instructor with the University of Colorado and prior to that a Clinical Instructor with the University of Missouri. Dr. Tatum was also a staff physician with a hospice in Colorado. In addition, Dr. Tatum was a visiting scholar with the Center for Palliative Studies in San Diego, CA. Dr. Tatum has received numerous awards and honors for teaching and research and maintains membership in many associations and societies. Dr. Tatum is fluent in Spanish.

Mel Hector, MD, CMD. FAAFP

Dr. Hector brings a wide breadth of experience to the TLC practice. He obtained his medical degree from the University of Missouri, is a Certified Medical Director, and is a past Clinical and Research Fellow in Geriatrics at Johns Hopkins Health Systems. He was also an Assistant Professor of Medicine at the University of Missouri. He is board certified in family practice and earned a Certificate of Added Qualification in Geriatric Medicine. Dr. Hector’s past work experience includes private practice, founding/leadership roles in medical groups/independent practice associations, and is a past Chief of Staff of Northwest Medical Center. Dr. Hector has been named twice in Naifel and Smith’s “The Best Doctors in America”. He maintains memberships in numerous associations and academies.

Jane Heaton, MD

Dr. Heaton is well-known for her compassionate and pioneering work in pain management and palliative medicine. She is board certified in internal medicine, has a Certificate of Added Qualifications in Geriatrics, and is board certified in Hospice and Palliative Medicine. She has been a long-time participant in the Carondelet Health Network in Tucson. She is a past-Medical Director of Carondelet Hospice Services and a past Board Member of Tucson Physicians IPA. Dr. Heaton has been in private practice previously and has spoken and written extensively on geriatric medicine, pain management, and hospice and palliative medicine. She has been an educator for the University of Arizona College of Medicine.

Margaret Brown , MD

Dr. Margaret Brown is a board certified family physician who joins TLC from her recently completed geriatric fellowship at the University of New Mexico. Originally from the Midwest, her medical practices have included venues in private family practice and emergency medicine in the West and Southwest, including several within Arizona. From 1991 to 1995, Dr. Brown served as a Medical Officer in the U. S. Navy. Within TLC, she will see patients in skilled nursing facilities in addition to various community settings under the Pain & Palliative Medicine Practice.

James L. Major, MD

Dr. Major obtained his medical degree from the University of Colorado, School of Medicine and has since become a board-certified internist with a Certificate of Added Qualifications in Geriatric Medicine. He joins TLC after almost 20 years of practice in Washington State in Internal Medicine and Geriatrics. He served as a medical director of several skilled nursing facilities during that time, and has served for years on a speaker’s bureau, addressing various topics in hypertension and cardiovascular prevention topics. His primary focus within TLC will be in the outpatient setting.

Christopher P. Marshall, MD

Dr. Marshall attended medical school at the University of Texas at San Antonio and completed his Residency in Family Practice at Texas Tech Regional Academic Health Center. He is board certified in family practice. Dr. Marshall’s background includes all aspects of family practice and some experience in urgent care. For fifteen years he worked for CIGNA HealthCare of Tucson and then operated as a solo practice physician for six years. Dr. Marshall joins TLC and will be seeing patients in the clinic setting.

Rise-Ann Wohl, MD

Dr. Wohl received her BA in Psychology at Vassar College in New York and then went on to earn her MD at New York Medical College. She completed an internal medicine program at Danbury Hospital in Connecticut and is board certified in internal medicine. She moved to Tucson in 1994 and has worked in the primary care field. Dr. Wohl joins TLC and will primarily be working in the TLC Medical Group – Catalina Foothills.


David Massanari, MD

Dr. Massanari joins TLC with years of experience and community service. He attended medical school at the University of Illinois and completed a residency in Family Practice at York Hospital in Pennsylvania. He earned the Certificate of Added Qualification in Geriatric Medicine in 1990. He serves as a Diplomat of the American Board of Family Medicine and is involved in a wide variety of professional organizations including his role as an instructor in Ambulatory Care and Prevention at Harvard Medical School. Dr. Massanari has published articles on a variety of topics including quality improvement, managed care and the role of the physician in primary care. In his role with TLC, Dr. Massanari will see patients at the skilled nursing facilities in Green Valley as well as in Tucson.

Karen M. Chandler, MD

Dr. Chandler is board certified in Family Medicine and in Hospice and Palliative Medicine. She comes to TLC from Winston-Salem, NC, where she was Associate Medical Director of Hospice & Palliative Care Center, and Medical Director of the Carousel Center for Pediatric Palliative Care. She also served as an Assistant Clinical Professor in the Department of Family Medicine at Wake Forest University Baptist Medical Center. Her previous practice experience includes primary care, long-term care, and public health. She speaks nationally on various topics, especially communication issues in palliative care, and pediatric palliative care.

Sreelekha Susarla, MD

Dr. Susarla attended medical school in Kakinada, Andhra Pradesh, India. She came to Tucson, Arizona in 2001 where she began the Tucson Hospitals Medical Education Program and completed an internal medicine residency. Dr. Susarla is a board certified Internist and was Chief Resident during her Internal Medicine Residency program at the University of Arizona. She then went on to work as a hospitalist in two local Tucson hospitals. For TLC, Dr. Susarla will be working primarily in the nursing home setting.
