Scott Bolhack, MD, MBA, CMD, CWS, FAAP, FACP, has been in practice in Tucson since 1992. He began his private practice in Catalina, Arizona, and maintains an outpatient practice in Oro Valley in the Northwest Medical Center at Rancho Vistoso. Presently, he holds the following positions: chief executive officer (CEO) of the Tucson Long-Term Care Medical Group, Inc; CEO of TLC Pain & Palliative Medicine Practice, Inc; president and CEO of TLC Medical Consulting Group, LLC; and president and CEO of TLC HealthCare™ Quality Improvement Organization, Inc. He is a clinical assistant professor with the Department of Medicine at the University of Arizona. Tucson Long Term Care Medical Group, Inc. was awarded the winner in the Business Growth Category 1-19 employees 6th Annual Wells Fargo Copper Cactus Awards in 2003; TLC HealthCare Companies now has over 55 employees.

Dr. Bolhack speaks nationally on the subject of rehabilitation in nursing homes and has gained extensive experience in the area of geriatrics. Dr. Bolhack has additional experience as a medical director for nursing homes, assisted living facilities, hospices, and home health agencies. He is a fellow of the American College of Physicians and a member of numerous professional societies, including the American Medical Directors Association, American College of Physician Executives, American Academy of Home Care Physicians, and American Academy of Hospice and Palliative Medicine.

He has presented over 20 scientific posters in the areas of quality improvement, pressure ulcers, and subacute care in skilled nursing facilities.

Dr. Bolhack earned his bachelor of science in biology from Bucknell University, his doctor of medicine from the Health Sciences Center of Brooklyn, and his masters in business administration from the Eller College at the University of Arizona. He is board certified in internal medicine, hospice and palliative medicine, and pediatrics. He has additional credentials as a certified wound specialist.

Dan Peterson, MBA
Vice President/Chief Financial Officer

Dan Peterson joined TLC in May 2003. Dan has an extensive healthcare background that includes prior experience at the health plan level, a large multi-specialty IPA, an academic medical group and a national hospitalist company. Dan has started companies, raised venture capital and grown entities into multimillion dollar ventures. In addition, Dan has taught economics and finance at the collegiate level. He has his Bachelor's degree in Health Services Administration from the University of Arizona and a Master of Business Administration degree from Pepperdine University.

Ilyn Nelson, MBA - Finance Manager

Ilyn received her Bachelor of Science in Business at Western International University in Phoenix, Arizona and a Master in Business Administration from Webster University in St. Louis Missouri. Ilyn comes to TLC with experience as an accountant/payroll manager for a large multi-state company and has worked for the State of Arizona in the Department of Revenue and the Retirement System. She has a strong background in all aspects of budgeting, accounts payable/receivable and human resources and will be responsible for the financial functions for TLC HealthCare™ Companies.

Shirley Grant, LPN

Shirley comes to TLC with 35 years of nursing experience. She has worked in a variety of health care settings, including long-term care, both as the Assistant Director of Nursing and as an assitant to the MDS Coordinator. She is the Administrative and Research Assistant for TLC Healthcare™ Quality Improvement Organization, Inc. Shirley has a special interest in comprehensive assessment issues and for improving the quality of care for the frail elderly in our skilled nursing facilities and assisted living facilities.

Paige Hector, LMSW

Paige Hector earned her Bachelor of Social Work and Sociology degree from Nebraska Wesleyan University, and then earned a Master of Social Work degree from Arizona State University. She has extensive experience in medical social work and has worked in many healthcare settings including acute care, long term care, adult day care and case management. Paige works for TLC HealthCare™ Companies with special focus on the Pain & Palliative Medicine Practice. She lectures on topics including pain and palliative medicine, end-of-life issues, advance directives, the Prospective Payment System (PPS) and the role of the social worker in the long-term care setting. Paige is a member of the National Association of Social Workers (NASW) and the American Medical Directors Association (AMDA).

Barbara Viggiano, RN, WCC

Barbara joined TLC in September 2004. The major focus of her nursing career has been in the care of the elderly. She brings with her a wide range of interests, including regulatory issues, quality assurance, MDS Coordination, and over 5 years experience as a nursing home Assistant Director of Nursing and Case Manager. This background gives her a unique understanding of the importance of quality care in healthcare settings. Barbara is also wound care certified and is available as a wound consultant to our providers and to our affiliates in the community.

Kathryn Derrick, RN, BSN

Kathy comes to TLC with a strong background of team building, case management and quality improvement. She has worked in the local health care environment for years, and brings a wealth of experience from both the side of the provider and the health plan. Liaison with managed care, skilled nursing facilities, utilization review activities and marketing strategies are some of the skills Kathy will put to use in her new role at TLC. The focus of her immediate efforts with TLC will be Quality Improvement activities and assistance in the opening of TLC's newest clinic in the Catalina Foothills this winter.

Kia Burgess, BSW, MBA

Kia comes to TLC with years of case management experience with a local HMO. Kia has extensive knowledge of the health care system at all levels and has worked with patients, family members and rehabilitation team members to develop care plans to best meet the needs of the patients. Kia has a Bachelor of Social Work from Arizona State University and a Master of Business Administration from the University of Phoenix. Her duties with TLC include overseeing communications, scheduling, working as a liaison with Evercare, and being a part of the TLC HealthCare™ Companies Quality Improvement Organization.
